1. On-site Registration hours: morning – 8:20 am ~ 11:30 am; afternoon – 1:20 pm ~ 4:20 pm
2. Clinic hours: morning – 9:10 am ~ 11:40 am; afternoon – 2:10 pm ~ 4:40 pm. Any official changes to
the OPD hours will be announced by the Registration Office.
3. Appointment & Registration by phone: 3366-2175 *Ophthalmology: Eyeglass prescription and optometry
services do not accept appointment by phone.
4. Appointment by phone: morning – 8:10 am ~ 9:00 am; afternoon – 1:00 pm ~ 2:00 pm
5. Check-in time for appointment: morning – before 10:30 am; afternoon – before 3:30 pm. Spots will
be reserved for those who made an appointment by phone (within seven days); numbers determining
order in which patients are seen will only be given after check-in. Latecomers’ spots will not be reserved.
6. On-site Registration: OPD morning hours are restricted to morning registration while afternoon hours
are restricted to afternoon registration.
7. Registration fee: NT$ 50 (free for faculty ID and student ID holders)
8. Partial liability fee: NT$50 (free for children under 3 years of age)
9. Deposit: NT$350 (for those with health insurance but forgot to bring their health insurance card).

10. Sports injury clinic for school staff and students every Monday and Thursday afternoon. Please make
appointment in advance to be evaluated by the family physician..
11. Abdomen ultrasound service every other Tuesday morning. Arrangement is made after evaluation by the
family physician.
